Spotlight on Caring: Yuna J.

Navigating the health care system can be challenging for so many. The opportunity to help members make the right care connections is an important part of the Optum® HouseCalls visit. All too often, members are unable to keep up with their primary care visits, or they don’t know where to go for help when significant time has passed. That was the case for this member, who needed help making the connection to essential care.

Yuna’s story

For some members, finding consistent health care can be challenging — even overwhelming. That was the case when HouseCalls advanced practice clinician Yuna completed a HouseCalls visit with an 87-year-old man. In talking with him, she learned that since his physician retired a year earlier, he had no direct connection to primary care. However, notes from the previous year’s HouseCalls visit indicated that the member was diabetic and had atrial fibrillation — two conditions that required ongoing monitoring.

After conducting the member’s physical exam, Yuna took action to help find him a new care provider. She helped coordinate with the member and their health plan resources to find a local provider who could see the member right away.

The health plan resource called local offices until they found one that could see the member the next day. Then, to help make sure the member was able to get to the appointment, with the member’s permission, Yuna contacted the member’s son, who agreed to help him get there.

Overall, the visit took a few minutes longer, but Yuna was confident that she had helped connect the member to the essential care he had been missing. The member was extremely thankful that Yuna could help him coordinate with his health plan to schedule the appointment — something he didn’t feel comfortable doing by himself.

What seems simple to some can feel impossible to many. Through the skilled efforts of Yuna and the health plan resource, this member was able to get the health care he needed to avoid what could have led to more serious complications.
