Consumer-centric wellness solutions for health plans

When individuals are able to proactively manage their health and wellness — whether they have multiple chronic conditions or are relatively healthy — they’re able to live their best lives. And as health risks are reduced, so are costs.

Optum offers NCQA-accredited wellness solutions tailored to the needs of each population’s needs. These services are delivered in one platform, providing a seamless member experience.

This platform also provides our  coaches and nurses with a 360-degree view of each individual, so they can coordinate care and provide members with the resources they need, when and where they need them.

Wellness Coaching

Supporting the well-being of your members helps you improve Star ratings and HEDIS measures. Our wellness coaches meet members where they are and communicate with them via their preferred channel, whether by phone, email or chat.

Coaching support is available for weight loss, exercise, nutrition, stress management, quitting tobacco, diabetes and heart health.

Targeted incentives and content

Incentives are targeted to the member’s goals. Interactive, multilingual, curriculum-based content supports  them in reaching those goals.

Health risk assessments online, by phone or on paper

Our comprehensive health risk assessment (HRA) identifies modifiable risks and educates members about their health status. Based on the individual’s needs, we recommend specific actions to improve health. We can refer them to external programs or Optum health programs.
